The Chemistry Joint Safety Team would like to inform the department of the new voluntary safety incident reporting program (http://jst.chem.yale.edu/safety-incidents). Reporting safety incidents and near-misses allows our community to learn from the mistakes of others, and decreases the chance of similar incidents happening again. Regardless of whether an incident occurred or whether it was prevented in time, we encourage everyone to report incidents that they believe others can learn from. The submissions are anonymous, and contact information is not required. We are collecting the submissions into a database that will hopefully serve as a resource for the department.
